Well, at the end of a trying day, I got some good news.
First, the trying part - I had perhaps the worst nightmare/flashback I have had in quite some time last night, and I woke up feeling shaky. Added to the fact I am fighting a flu, and it was not the best way to start my day.
But now for the good news. I saw my gender specialist today, and for the first time in months, I have hope again that a surgery could be in my future. He said he will contact a doctor in Arizona and see if my weight would be an issue for him, and if not, he will lobby the government for funding. And if he says no, he will get me a orchidectomy to remove my testes, so at least I would be that much closer to being fully complete.

GRS is the accepted cure for gender dysphoria. No offense, but don't listen to Katie. Katie apparently has some idea about your psychology and what's good for you. Katie might be thinking about an actual case with actual problems, but Katie doesn't have a clue about who you are, what you can stand and what would be good for you compared to the therapists you've already talked to.
If you come to the Phoenix area, because the only GRS surgeon I know of out here is in Scottsdale, I can help you. I can pick you up at the airport, drive you where-ever and visit you while you recover. The only other good medical area is in Tucson; if you go there, I can at least visit you.
Why don't you ask your specialist for a referral to a weight loss therapist? Maybe you could work out why you've been unable to lose (enough) weight.

There are many good doctors
out there. I'm not sure why you would pick an out of country surgeon, but it does not matter. Get er done girl.
My SRS doesnt hurt a bit until I stand up. Then it is {insert any wods you like here}, till I lay back down. Given how much I love Christmas I'll endure some pain to be with my friends and family. It is worth it. It was worth it. It will be worth it!
