For the second day in a row I'm in terrible pain.

Enough pain to go past "crying" and head toward "screaming " ...

I don't know where you are, but that qualifies as a legitimate reason to go to an Emergency Room, at least in the US.
Get there, if unable to drive yourself, call for transportation. Don't be stoic !

upper back pain that is chronic, and now added to it is bursitis in my right shoulder that frequently has me incapable of using my right hand.

get the joy of the cortisone shot into the shoulder on the 29th.

and I hate needles.

Been there. I have permanent nerve damage which sends false pain signals to my brain, but it still hurts. Go to the hospital or see your doctor. (I wound up in the hospital for 5 days.)

I have to normally resort to cold and heat

I use cold packs, and a home-made hot pack made from cloth and filled with dear corn and I can sometimes get enough relief to not be in agony.
