Shelly and I have had problems with doctor's offices just not sending paperwork on as promised.
From what you said, it may just be a case of you needing to get on their case.

My family doctor lost my paperwork? Dorothy, if they bill you for the physical, demand the results, or a new physical for free! If refused, sue for malpractice. I am sure that an attorney could win the case, especially with a judge who supports you.
