She told me that in order for her to write a recommendation to a doctor, we need to have a good deep discussion about my gender identity? Does anyone have any suggestions for topics I should think about my answers to before hand and bring up with her on Wednesday.

I feel I should point out that I've been seeing her for a while, and that she said we were going to discuss more about my being transgendered.

this should be self evident

You shouldn't be preparing for her questions... you should be loosening up your inhibitions and preparing for talking that hurts your whole body to say it out loud. I remember my first day... it felt like I'd ripped my chest open to expose my ugly bits and it continued to hurt for hours afterwards... mostly due to my crying.

ps. that said ask her about her process. how does it usually work. If she finds you are TS then what will she do to help you along the process. Who does she work with and recommend for the 2nd opinions. Are there any GPs she can recommend.

my first therapy session

I was just honest. It wasn't really ripping my heart out or nothing, it was just stating what I knew. The real fear is that you don't be yourself and try to follow the party line of being TG. Therapist can see right through that. Talk about yourself. Talk about how you view yourself. From what I read and hear, my GID doesn't follow the normal pattern (i.e. I didn't KNOW I was a girl from the fetus and wasn't fighting the male stereotype my whole life). The more honest you are, the more it is going to help you in the long run. The goal of therapy is not to give you what you want, but what you need (sometimes they go hand in hand).

Heart, mind and soul

If you have to practice your answers, and need a cheat sheet on what to say, your may not be ready to go further in your transition. You need to be able to tell the therapist, what makes you a girl, and not the boy you appear. It needs to come from you mind, your heart, and your soul (your deepest inner being). You need to leave all inhibitions behind. It's her job to be hard on you.
