okay so last night my CPAP machine was going at such a pressure it actually caused the hose to separate from the mask.

I'm gonna go out on a limb and say that's not good.

What pressure is it set at? (Number of centimeters of water?)

Pretty much if it comes off, put it right back on. You may have to push it hard over a few jags.

I suspect that it was inching in that direction for some time, and finally reached the point.

Mine is at a medium pressure (12-14) and it sometimes comes off. Usually when I am having a restless night. The hose gets caught under an arm or a pillow and moving around gets it pulled off. I've reduced the occurrences by routing the hose from behind the bed through my slatted headboard instead of from the side. Experiment until you're comfortable. I've had to make many adaptions over almost 30 years and can usually manage to make it work.
