Is there an alternative?
Clearing the catheter is causing problems. I hope you can find an alternative which is not so problematic.
I know that fight
I know well the trials of living with a indwelling cath all the time.
Your bladder is most likely not happy having anything in it like that, especially a foreign object that "doesn't belong" in there and it is probably reacting like a overactive bladder. Hence the cramps when your bladder is trying to eject the cath.
Here's what I can suggest off the top of my head from personal experience.
First thing I'll recommend is making sure that you are drinking plenty of water - not fluids - water. You should be passing a bare minimum of 3000 ML per day, and the constant flow through your body will help keep the bladder somewhat passive. Second is, what size balloon do they have you using? If they started you out with a 30cc balloon and you have been using that size all along, then it may be time to go down to the 10 CC balloon. That will make your bladder happier having a smaller size inflated balloon in there.

I hope things improve soon.
One of the problems with long term catheterisation is the incidence of UTIs, so make sure that's not happening.

Second that. Please check for it.
