Sissie/Tomboy
I always took it as being like a tomboy, only the other way around -- the main difference being that tomboys are tolerated/respected a whole lot more than sissies. It wasn't until later that I heard the term used in a non-derisive fashion.

Sissies and Tom boys
I'm not really sure that the connotation of a sissy referred to a boy at first. I believe there were little girls who were ultra feminine and they gained the nickname sissy. Tom girl sounds like it came about around The time Tom Sawyer and Huckleberry Finn were written. So why can't there by a more definitive name for a boy who is effeminate Like Jane girl.
