It's about evaluating the risk
Is it worth it? What could you lose? Let's face it at 90, unless your family are very upset, there isn't much to lose except the oestrogens would incur a certain amount of risk.

I think at 90 I'd be content with listening to Enya, and wondering if the tulips were going to bloom in winter.

My muse suffers from insomnia, and it keeps me up at night.
And why not ?
At 78 years of age I was depressed , angry and suicidal with severe PTSD from years as a Paramedic and from sexual abuse as a 14 year old . My lovely counsellor Lyn saw through me and persisted until I finally told her of my gender problems and now at 84 I enjoy a serenity and happiness I never knew in my previous existence .Sure, problems with some of the older family members but that is their problem ,not mine .
My four grand daughters (despite their mother ) are most accepting as are two nieces and two female cousins . I did have problems with estrogen initially (tablets ) and ended up with Pulmonary Embolism but that was corrected with injectables which are unfortunately difficult to access down here in Australia but I am having good results from Sandrina Gel in the interim . A lovely GP (of Irish extraction) looks after me and is very switched on unlike the previous GP who had to have the difference between Gender and Sexuality explained to her . I don't think it is ever "too late " to have happiness and I have to thank so many girls here on BCTS who have supported and encouraged me .

Transgender MtF live longer
I am told that I will likely live 15 to 20 years longer than before. All of my relatives are dead except my son, and he has had kidney cancer, though it may have been occupational. In males, apparently the loss of testosterone and the addition of low dose Estrogen makes you more healthy.
Of course that presupposes that there is not smoking or heavy drinking; a managed body weight and a healthy diet.
