A published gender changing book
Try "Steel Beach" by John Varley, I enjoyed it and is available @ Amazon.com (but look around for a better price).

However, the theme is upbeat, near-future SF, not transgender issues (although these do sometimes form part of a story: check out a story I've got lined up for Christmas Day called "Fembot").
